One of the questions asked most frequently in the social security office is “What proofs will I need when I file an application for social security benefit?’’. Mrs. Hazel F. Daly, manager of the social security office in Winchester, Virginia stated that the required proofs vary in some cases, however, she indicated that the following proofs are needed in every case:
1.Social security card for the worker and for any member of his family who is filing for benefits.
2. Proof of age for the worker and his wife if she is at; least 62.
3. Proof of age fol^his children under age 18 or over 18 if they became totally disabled before they attained age 18.
4. Proof of earnings for the year .firfiwe application 4s -filed.' This will be Form W-2 if the worker is employed by some one, other than himself. If he is selfemployed, he will be required to furnish a complete copy of his income tax return for the previous year and he will also be required to furnish proof that he paid the social security tax. He will also be asked to estimate the amount of his earnings for the year in which he files application for benefits.
5. Widows who are filing on the record of their deceased husbands will be required to furnish a marriage record.
While Mrs. Daly emphasized the importance of furnishing the inecessary proofs, she indicated that a person should not delay filing an application simply because he is unable to obtain a certain proof. The employees in the social security office will assist persons in any way possible in obtaining the proofs that are needed.
