Veterans and dependents of veterans on the Veterans Administration's Compensation and Pension rolls who have school age children, will [receive a special questionnaire with their January checks F.dward M. Mathews, Claims' Agent of the Division of War Veterans’ Claims, announced this week. Mr. Mathews said the checks and questionnaires would arrive in their mail boxes about February 1, 1966. Need for the information asked by the questionnaire follows a recent law that provided higher rates of additional payments for children of veterans and dependents, who attend school; and, at the same time raised the maximum age limit from 21 to 23. The questionnaire also asks for the payee’s zip code number in keeping with current postal regulations in order to speed up the delivery of both checks and correspondence. Also, a space is provided for the payee's social security number, so identification files may be kept up to date. The questionnaire should not be mutilated nor folded and all questions should be answered before the card is signed. A special addressed envelope is provided for' the return of the questionnaire. Veterans or dependents who need assistance or have questions about the questionnaire are invited to call on any office of the Virginia Division of War Veterans’ Claims Mr. Mathews is in Washington at the clerk’s Office on 1st and 3rd Monday from 2 to 4 p. m.
