Missing signatures and addresses are two frequent prob lems that Travelers Insurance Co. has run into in processing doctor bill request for payment forms from medicare beneficiaries. Hazel F. Daly, social security district manager in Winchester said today. Representatives of Travelers, the carrier for medicare doctor bill payments in this area, have reported that some beneficiaries have received repayment for doctor bills later than was necessary because they forgot to sign their forms or to include their address.
The signature on the request for payment form authorizes the carrier to pay the claim. Travelers Insurance Co. has to have the signature before the claim can be paid. A missing address presents even a bigger problem according to Mrs. Daly. "WithW
