23208. Application forms may be obtained from local health departments or a letter may be sent to the Bureau giving the child’s full name, date of birth, place of birth, full name of father and full maiden name of the mother.
In all cases, requests should be accompanied by a fee of $2 for each copy desired. Checks and money orders should be made payable to the State Health Department.
If the child was bom in another state, the request should be sent to the Vital Statistics Office in that State’s capital city.D
