All children must be accompanied by an adult and should have available the birth certificate and verification of immunizations.
The Public Health Nurse will be at each school as scheduled and will make appointments for those parents desiring health examination and immunization at the Health Department.
All parents will be supplied with the necessary blanks to be completed and returned to the schools by May 15, 1964. These blanks include health examination by a physician, immunization for smallpox, and diphtheria, registration blank and completion of pupil jslactfitfeftt blank.
