Any condition, disease or ailment which existed on.or before the effective date of membership; b. Hernias, tuberculous conditions, nervous and mental conditions;

c. Any elective surgery.

SENIOR CITIZEN CONTRACT DOES NOT COVER: a. *ln-patient care required primarily for diagnostic purposes;

b. In-patient care which could just as wellJiave been given in the home, doctor's office or an out-patient facility;

c. Home or doctor's office care, except for accidents or surgery;

d. Care provided for under Federal, State or local laws, including Workman's Compensation Laws.
