A “yes" answer to any of these questions may mean loss of wage credits unless you notify your nearest social security office. Your name on the payroll should be the same ms the one on your social security card. If your name has been changed since you .got your card simply fill out a “Request for Change in Records” form. You can get this form at the social security office or your local post office. A new card will be furnished free. It will show your correct name with your original number.
If your card has been lost or worn out mail the stub to the social security office and a duplicate card will be sent to you. If you do not have the stub you can still get a new card by just filling out an application. Your new card will bear the same social security number you had before.
Don’t ask for more than onenumber. If you should have more than one number you might have trouble getting all of the social security benefit payments that will bo duo to you and your family
