In June, letters went out to approximately 140 parents of 7th-11th graders in Rappahannock County advising them that their child’s immunization record was incomplete. As of July 31 only 35 have been returned.
In order for the remaining students to attend classes in the fall, it will be necessary for the parent or guardian to complete the form and return it to the high school. If records at home show that immunization has been received, it will simply be necessary to complete and return the form. It is not necessary in this case to visit the doctor for her signature. If, however, records are not available at home, it will be necessary for parents to contact their doctor or the health department for information.
