Documents which will establish a young child’s identity, and which are usually available to most families, include: report card, schoolr identification card, baby’s hospital wrist band, newspaper birth announcement, baby book, court order for name change, church membership or confirmation record, doctor or hospital records, vaccination certificate, daycare or nursery school records, or child’s membership in Boy ScoutSj Girl Scouts, or other young organization.
This is not an exclusive list, Hundley said. Any document providing identifying data such as a physical destription, photograph, or signature may be used as evidence of identity.
