Position requires formulation of service objectives and identification of actions needed to assist clients in utilization of resources and capabilities. Under general supervision, this position will provide prevention services to at-risk families. Position plans and organizes caseload, establishes priorities, schedules meetings and/a appointments; and maintains documentation for each participant in accordance with the program. Works directly with family to provide case management services. Position may require travel to participant's home and telephone contact Position requires preparation of correspondence. Applicant must possess a minimum of a baccalaureate degree in the Human Services field or possess a minimum of a baccalaureate degree in any field accompanied ty a minimum of two (2) years of experience in a Human Services related area Criminal record and Chid Abuse & Needed Information System background checks wit be required.
Salary Range: $29,876 - $31,090 plus benefits
