A registrar of voters will be available on the following days, for the purpose of visiting, to take the applications to register to vote of qualified persons who are unable to apply for voter registration in person at the office of the general registrar, due to a continuing illness or physical disability that has existed for at least thirty days:

SEPTEMBER 30.1994 Forms for requesting such a visit may be obtained by writing to the address shown below or by calling 703-675-3739 on Monday - Friday between the hours of 8:30 am - 4:30 p.m. A properly completed form must be received by the general registrar not less than five days prior to any approved day shown above in order for a visit to be scheduled on that day.
