The most typical way this is done is through home exercise programs. A home program may include strengthening, range of motion, exercise or possibly retraining of the performance of an activity.

More and more of the responsibility of the physical therapy conditioning program is placed in the patients' hands while the therapist concentrates on the manual treatment techniques and patient instruction during die office visits.

The patient instruction and subsequent implementation of exercise and/or behavior modification is essential in order to achieve treatment goals. This may include the proper use of modalities (heat, ice, aquatics), proper body mechanics, posture training, transfer techniques, gait modifications, orthotic implementation and’, most commonly, strengthening and ROM exercises.

If the patient is going to be able to transfer the information outside clinical setting, the physical therapist needs to be an effective teacher and communicator. This requires a thorough understanding of the patient's diagnosis and experience in the appropriate treatment techniques.
