Select new or renewal. / Fill in six numbers OR Easy Pick if you want the computer to select your numbers. These will be your Lotto numbers for your entire subscription; you can’t change these numbers until you renew your subscription. / Fill in the subscriber’s name, address, birth date and other information as required on the form. The Social Security # is only required when a subscriber wins, so you can leave it blank for now, if you wish. The subscriber could be you or a friend.
