Moten found “no evident sign of burning, bulging or distortion of the boiler" following the pressure buildup on May 7 and 8.
According to the fire marshal, "one reason the boiler overheated was that an automatic control for the heating system had been bypassed because of a malfunction in the control.”
In order to prevent repeat occurences of the same type of situation, Frese suggested that a staff policy be established "to require immediate replacement of control mechanisms when the heating system is in use until the control mechanism could be repaired or replaced.”
