Responsible for working closely with LCPS Transition Teachers to identify students with mental retardation exiting Loudoun County Public School system, making referrals to appropriate service providers, linking students and their families with community resources and managing a caseload of students currently receiving services. Requires direct experience working with public school system transitioning students into adult services, general knowledge of IEP process, basic understanding of funding sources, general understanding of CSA/FAPT. knowledge of eligibility process for state/federal benefits and knowledge of community resources. Able to demonstrate effective communication orally and in writing, be able to make presentations to groups. Bachelor's degree in psychology, social work or related field and two years directly related experience. Criminal background check, driving record check.
