Provide case management services for MH consumers and their families Will provide intensive supports to a small caseload of individuals placed in both 24 hour and supported residential programs in Rappahannock. Duties include providing assessment, advocacy, case management, consultation and training for consumers and their Families in outpatient, inpatient, emergency and day program settings. Knowledge of service delivery systems and types of mental health programs and services. Demonstrated ability to deal sensitively with consumers and Families and to communicate effectively. APARTMENT LIVING BESIDENXAL COUNSELORS MM Full-time non-exempt positions under the supervision of a Supported Living Program Manager to support adults with mental illness and mental retardation. Provide assessment living skills training and support for consumers in new residential group borne and/or independent living in apartment settings. Evaluation and asses consumer skills and develop individual naming plans, assist with and mom tor consumer tadgets and provide payee services and needed, provide shopping and cooking assistance, plan and participate in recreational activities. Provide support through feedback through direct contact in areas of interpersonal skills, neighbor and landlord relations, etc. Report to primary therapist on mental status and issues and participate in consumer disposition meetings. Knowledge of community, bousing and financial resources for the chronically mentally imparted, skills in assessment of consumer's function capwnty and demonstrated ability to deal effectively with consumers. SUBSTANCE ABUSE TECHNICIAN Hourly and on-call positions available at residential substance abuse treatment facility to monitor activities of consumers and ensure their safety and welfare. Demonstrated ability to work with substance abuse population.
