Position is responsible lor coordinating, assessing, monitoring, and linking service delivery for consumers with disabilities and their families. Responsibilities will focus on coordinating and monitoring support to assist in community integration for these individuals. Requires education and experience equivalent to a Bachelor's degree in psychology, social work, or related field and two years job related experience. Considerable program experience with mentally rciarded/developmentallv disabled persons is preferred. VA Medicaid waiver experience is helpful.
