AGING PROGRAM SPECIALIST

The Department of Parks, Recreation and Community Services, Area Agency on Aging, is currently seeking qualified applicants for an Aging Program Specialist position. This position performs intake assistance, which includes providing appropriate information and referral/assistance to clients in the aging population. Assesses the needs of clients and families. Coordinates the Disease Prevention and Health Promotion Program. This reliable and detail oriented position coordinates transportation for seniors to attend doctor's and other important appointments. Develops and maintains confidential client records using automated systems. Provides support and training to volunteers and education and community outreach activities for older adult population. Successful candidate must have strong communication, customer service, organizational and time management skills. Requires any combination of education and experience equivalent to Bachelor's degree in Social Work, Gerontology or related field, with 2 years experience working with the elderly or special population. Prefer experience providing assessment and case management services. Valid drivers license required. Experience in database mariagement and word processing is preferred. Requires regular, sensitive interactions with clients with mental and/or physical impairments. Must be able to respond to client emergencies requiring contact with emergency response systems.
