On September 1, 1982, Central Telephone Company of Virginia ("Centel" or "Company") filed an application for increased rates under the Commission's Financial Operating Review ("FOR") procedures Centers application for increased rates was in two parts, or "phases " "Phase I" contained prescribed FOR data and adjustments supporting rates intended to generate $2,269,533 in additional annual revenues by means of a 5 77 percent surcharge applied to most current Centel tariff rates. Centel proposed to place "Phase I" rates into effect, on an interim basis, subject to refund, 30 days after the September 1, 1982, filing date of the application "Phase II" of the application contained additional data supporting rates which Centel proposed to place into effect on January 1, 1983 The proposed "Phase II" rates were intended to generate an additional $2,954,771 in annual revenues by means of a 13.59 percent surcharge (replacing the proposed. 5 77 percent surcharge)
