CLINICAL CARE COORDINATOR

Temporary, FT position. Provide intensive, community-based care coordination to select youth and families receiving services through Comprehensive Services Act/Family Assessment and Planning Team. Services include assessment and evaluation; case management, facilitating family team meetings, coordinating implementation of service plans; crisis intervention and stabilization; and interagency consultation and coordination of services. Maintains all clinical records and documentation according to federal, state and agency policies and procedures. Knowledgeable in mental health and substance abuse issues and treatment; advanced level of clinical experience in working with families; strong organizational skills; ability to work independently; ability to function effectively in an interagency capacity. Bachelor's degree in social work, psychology, counseling and/or marriage and family therapy and three years of job-related experience. Master's Degree in social work, psychology, counseling and/or marriage and family therapy, and three years post-masters clinical experience preferred; Virginia Clinical licensure or license-eligible.
